Please see below all the forms necessary to begin the Admissions & Registration process at St. Augustine School. 
Start with the Registration Checklist, it has a complete list of ALL the documents you will need to assemble to register your child/ren.  
PLEASE be aware that an INTERVIEW with the Principal, Sr. Mary Elizabeth Donoghue is REQUIRED before ANY applications are approved.  

You may fill out and gather all your documentation to bring to the appointment in advance, but you MUST meet with the Principal before any application is officially approved.

5. Transfer of Student Records for Kinder--8th grade only.  This authorizes SAS, with your permission, to receive your students records from their current school
6. Home District Authorization form.  Authorizes SAS to receive Library Materials, Textbooks,  etc for your student.
